Please know that if at any time you are uncomfortable with providing personal information online, you can always call our information line to schedule an appointment.

After requesting an appointment online, it may take up to 24 hours for us to contact you and confirm your appointment time and date. If you have not heard from us after 24 hours, please contact us. We are not open on Sundays, so any requests sent on Sunday for Monday appointments may not be confirmed or scheduled. Therefore, if you would like a same-day appointment, or you do not see an appointment time that you would like, we suggest that you call us directly to schedule your appointment.

Online appointments are for abortion services only.

Please know that we do not confirm fees when making an appointment online. Upon confirmation of your appointment, we will verify the details of your preferred payment method. If you need to know the price before you make the appointment, please call us to schedule.

Confidentiality Notice

To make an appointment online, we ask that you answer confidential health and personal information that is needed to complete your appointment. The information that we ask of you is no different than what we ask over the phone. Your information will be transmitted by secured, encrypted e-mail. Though we make every effort to keep your information secure, it is still possible that the information you provide could be captured by individuals who are engaged in illegal internet observance. By providing and submitting your confidential information, you accept the risk of sending this information by e-mail to Family Planning Associates Medical Group personnel. When submitting by e-mail, you also agree not to hold Family Planning Associates Medical Group, including its employees or agents, liable for any damages you may incur as a result of the transfer or use of this information.
